Root Cause Analysis

Creating effective instruction begins with creating a needs assessment. One step of this process to determine the main cause of a performance issue is to conduct a root cause analysis to discover the contributing factors.

In LT7150 my group looked at the problem of injuries occuring in school makerspaces. We conducted a root cause analysis to determine the cause of these injuries, particularly those happening with woodworking equipment.. To conduct this analysis we used the 5 Whys Method, Fishbone Diagram, and Role Playing.

I worked with Whitney Bailey and Crystal Rose on this project.
